Items are coded under HCPCS Level II, billed to one of four DME Medicare Administrative Contractors (DME MACs), and may be subject to Competitive Bidding pricing in designated areas. Suppliers must be enrolled, bonded, and accredited under 42 CFR 424 Subpart D.
