Core Laboratory

Definition
A core laboratory (core lab) is an independent facility contracted in a clinical trial to perform centralized, standardized acquisition, processing, and interpretation of study data, most commonly imaging such as angiography, echocardiography, or CT/MRI, but also ECG, pulmonary function, or other physiologic measurements. By analyzing all subjects' data using a single set of trained readers and a common protocol, a core lab reduces the inter-site and inter-reader variability that would occur if each investigational site's local personnel interpreted their own images, and it allows readers to remain blinded to treatment assignment and clinical outcome, which protects against assessment bias in device trials where blinding of the intervention itself may not be feasible.What this means in practice
Core labs are particularly important for medical device pivotal trials submitted to FDA under a PMA or de novo pathway, where the primary effectiveness endpoint is often an imaging-based or physiologic measurement, such as minimal lumen diameter in a coronary stent trial or ejection fraction in a structural heart trial. FDA's investigational device exemption and PMA review teams frequently expect core lab charters that pre-specify reader qualifications, adjudication procedures for reader disagreement, and independence from the sponsor to be documented in the study protocol and included in the submission.- •Allowing site-level image interpretation to substitute for core lab adjudication of a primary endpoint, which introduces variability that reviewers may flag as a study limitation.
