    Reimbursement

    MolDx Program

    In one line
A Medicare molecular diagnostics program, administered by Palmetto GBA and adopted by several Medicare Administrative Contractors, that technically assesses molecular tests and assigns Z-codes for claims processing and local coverage.
    Definition
The MolDx (Molecular Diagnostic Services) Program is a Medicare Administrative Contractor initiative, originally developed by Palmetto GBA and now used by multiple MACs including Noridian and CGS, that establishes coverage, coding, and pricing methodologies specific to molecular and genomic diagnostic tests billed to Medicare. Laboratories register their tests with MolDx and receive a unique test identifier known as a Z-code, which is required for claims submission and allows the program to track individual proprietary tests separately from generic CPT codes that would otherwise obscure which specific assay was performed. MolDx conducts a technical assessment (TA) of each registered test's analytical and clinical validity, and, where a Local Coverage Determination (LCD) exists for that test category, the technical assessment findings feed into whether Medicare will cover the test for its Medicare Administrative Contractor jurisdiction.
    Why it matters
For molecular diagnostic and genomic test developers, MolDx registration and Z-code assignment is typically a prerequisite to reimbursement in MolDx-participating MAC jurisdictions, functioning alongside, and sometimes overlapping with, PAMA private payor rate reporting and FDA regulatory status. Because MolDx LCDs are jurisdiction-specific rather than a single national policy, a test's coverage status can differ across MAC jurisdictions until CMS or the MACs harmonize policy, which laboratories must track separately from FDA clearance or approval status.
    Common pitfalls
    • •Assuming a Z-code alone guarantees Medicare payment; the code enables claims tracking, but coverage still depends on whether an applicable LCD covers the specific test and clinical indication.
    • •Treating MolDx technical assessment approval as equivalent to FDA clearance or approval; MolDx TA is a payor-driven evidentiary review distinct from FDA's premarket regulatory pathways.
    • •Overlooking that MolDx policies vary by MAC jurisdiction, so a test covered in one region may lack an active LCD, and therefore reimbursement certainty, in another.

Why compare MedTech terms side by side?

MedTech terminology is full of pairs that look interchangeable but carry very different regulatory, clinical, and commercial consequences. Picking the wrong framework, pathway, or standard early in a project can add months to a submission, invalidate clinical evidence, or trigger an audit finding. Side-by-side comparison is the fastest way to surface those differences before they become costly mistakes.

Each comparison on this page pulls from the same vendor-neutral, sourced definitions used throughout the MedTech Terms glossary. You see the one-line summary, the formal definition, why it matters in practice, common pitfalls, and the primary sources (FDA guidance, EU MDR/IVDR articles, ISO/IEC standards, MDCG documents, IMDRF principles) that back each entry. That makes the comparison defensible in regulatory strategy memos, design reviews, and submission narratives.
