# Medicare Physician Fee Schedule

Medicare's payment system for physician professional services.

Reviewed by [Christian Espinosa, Founder, Blue Goat Cyber](/authors/christian-espinosa) Last reviewed May 5, 2026

## Definition

MPFS pays physicians and certain non-physician practitioners using a Resource-Based Relative Value Scale (RBRVS) - each [CPT](/terms/cpt-codes) code carries Relative Value Units (RVUs) for work, practice expense, and malpractice, multiplied by an annual conversion factor.

What the regulation says

The Medicare Physician Fee Schedule (MPFS) is administered by the Centers for Medicare & Medicaid Services (CMS) and dictates reimbursement for physician services, as outlined in the Social Security Act. It is updated annually through rulemaking, generally published as the Medicare PFS Final Rule, which includes updates to the Relative Value Units (RVUs) and the conversion factor. While not a direct MedTech regulation, understanding the MPFS is crucial for manufacturers because it influences market access and adoption of medical devices and diagnostics.

## What this means in practice

RVU values for new device-enabled procedures shape physician adoption economics. The annual PFS rule and AMA RUC valuations are critical.

## Examples

- A company developing a novel surgical robot needs to understand the RVUs assigned to the CPT codes for procedures performed with their device, as this impacts surgeon reimbursement and adoption.
- Before launching a new diagnostic test, a MedTech firm analyzes the existing MPFS reimbursement for similar tests and anticipates how their new test might be coded and valued.
- A manufacturer educates physicians on the appropriate documentation for new device-enabled procedures to ensure accurate CPT coding and optimal MPFS reimbursement.

Common pitfalls

- • A common pitfall is assuming that device reimbursement directly follows the MPFS, when in fact the MPFS primarily covers physician services, not the device itself.
- • Misunderstanding the annual update cycle can lead to outdated financial projections for market entry of new technologies.
- • Failing to engage with the American Medical Association (AMA) Relative Value Update Committee (RUC) process can result in unfavorable RVU assignments for novel procedures utilizing MedTech products.
- • Another mistake is neglecting the impact of MPFS on physician practice economics, which can affect their willingness to adopt new device-enabled procedures.
- • Manufacturers sometimes overlook the difference between coding, coverage, and payment, where MPFS specifically addresses payment for physician services encoded by CPT codes.

## Frequently asked questions
