# Minimal Clinically Important Difference

## Definition

MCID anchors interpretation of patient-reported outcomes and many functional endpoints. Pre-specifying MCID is increasingly expected by FDA and HTAs to interpret trial significance. 

What the regulation says

Regulators like the FDA increasingly expect trial sponsors to pre-specify the Minimal Clinically Important Difference (MCID) in clinical trial protocols. This helps in interpreting the clinical significance of patient-reported outcomes (PROs) and other functional endpoints, moving beyond mere statistical significance. The  [EU Medical Device Regulation](/terms/mdr) ( [MDR](/terms/mdr-reporting)) also emphasizes the need for clinical benefit, which often involves considering MCID in assessing device effectiveness. 

## What this means in practice

Especially relevant for orthopedic,  [neuromodulation](/terms/neuromodulation), and  [digital therapeutic](/terms/samd-dtx) trials with PRO-based endpoints. 

## Examples

-   A manufacturer of a new orthopedic implant specifies an MCID of 10 points change on the Oxford Knee Score in their clinical trial protocol to demonstrate meaningful improvement in patient function. This pre-specified value guides the interpretation of the trial results regarding the implant's clinical benefit.
-   For a digital therapeutic aiming to reduce chronic pain, the sponsor defines an MCID as a 30% reduction in pain intensity on a Visual Analog Scale (VAS) to be considered clinically meaningful for patients.
-   An innovator developing a neuromodulation device for tremor identifies an MCID of 2-point improvement on a specific motor function scale, ensuring that the observed effect is perceptible and beneficial to the patient's daily life.
