# Inpatient Prospective Payment System

Medicare's payment system for inpatient hospital stays.

Reviewed by [Christian Espinosa, Founder, Blue Goat Cyber](/authors/christian-espinosa) Last reviewed May 5, 2026

## Definition

IPPS pays acute-care hospitals a predetermined rate per discharge based on the assigned [MS-DRG](/terms/ms-drg). The annual IPPS rule updates rates, NTAPs, and DRG reassignments - closely watched by MedTech.

What the regulation says

The Inpatient Prospective Payment System (IPPS) is a Medicare payment system for acute care hospitals, established by the Centers for Medicare and Medicaid Services (CMS). It sets predetermined payment rates for inpatient stays based on Diagnosis-Related Groups (DRGs), as outlined in 42 CFR Part 412, Subpart D. Each year, CMS publishes a proposed and then a final rule updating payment rates, policies, and DRG classifications, which directly impacts hospital reimbursement for services, including those utilizing MedTech products.

## What this means in practice

Comments to the annual IPPS proposed rule are a key advocacy lever for new device coverage and payment.

## Examples

- A MedTech company develops a novel surgical implant, and successful advocacy during the IPPS rulemaking process leads to its inclusion in an existing MS-DRG with an appropriate weighting, ensuring adequate hospital reimbursement.
- A new diagnostic imaging device receives a New Technology Add-on Payment (NTAP) designation, providing hospitals with additional reimbursement for its use in qualifying cases for a limited period.
- A hospital meticulously documents the severity of a patient's condition and the specific procedures performed, leading to assignment to a higher-paying MS-DRG for a complex MedTech-enabled surgery.

Common pitfalls

- • Hospitals might not fully understand the specific documentation requirements for optimal MS-DRG assignment, potentially leading to under-reimbursement.
- • MedTech manufacturers may over-estimate the impact of a new technology add-on payment (NTAP) without considering the broader IPPS payment structure.
- • Failing to engage in the annual IPPS rulemaking process can result in missed opportunities for advocating for appropriate reimbursement of innovative devices.
- • Misinterpreting the nuances of DRG assignments can lead to inaccurate financial forecasting for MedTech products used in inpatient settings.

## Frequently asked questions

How does IPPS affect MedTech companies?

IPPS directly influences the adoption and market access of MedTech products by determining how much hospitals are reimbursed for procedures where these devices are used. Favorable DRG assignments or New Technology Add-on Payments (NTAPs) can significantly improve product uptake.
