# Group Purchasing Organization

Entity that aggregates hospital purchasing to negotiate prices with suppliers.

Reviewed by [Christian Espinosa, Founder, Blue Goat Cyber](/authors/christian-espinosa) Last reviewed May 5, 2026

## Definition

GPOs (Vizient, Premier, HealthTrust) aggregate the purchasing volume of member hospitals to negotiate price and contract terms with suppliers. Most U.S. hospitals belong to one or more GPOs.

What the regulation says

While not directly regulated, Group Purchasing Organizations (GPOs) influence MedTech market access and pricing, which can indirectly affect compliance with quality system regulations like 21 CFR Part 820 (regarding purchasing controls) and EU [MDR](/terms/mdr-reporting) (regarding economic operators). MedTech manufacturers must understand GPO contracting to ensure their products remain available with appropriate quality and safety documentation.

## What this means in practice

GPO contract status (on-contract, dual-source, sole-source) materially affects MedTech pull-through. GPOs also operate consulting, analytics, and clinical evidence services.

## Examples

- A MedTech manufacturer develops a new surgical instrument and must navigate GPO contracts to gain access to hospital systems that rely on GPO-approved vendor lists for procurement.
- A hospital group, through its GPO, negotiates a contract for a specific type of implant, requiring the manufacturer to provide detailed evidence of regulatory compliance and post-market surveillance data to secure the agreement.
- A small MedTech startup aims to introduce an innovative diagnostic device and seeks GPO inclusion to accelerate market penetration and build trust with potential hospital customers.

Common pitfalls

- • MedTech manufacturers might assume GPO inclusion guarantees market adoption, but individual hospital purchasing decisions still prevail.
- • Underestimating the time and resources required to secure and maintain GPO contracts can lead to missed market opportunities.
- • Failing to consider the GPO's influence on supply chain stability can impact a manufacturer's ability to consistently meet demand and regulatory obligations.
- • Manufacturers might overlook the need to align their quality management system processes, such as supplier management, with GPO purchasing requirements and specifications.

## Frequently asked questions

Do GPOs create regulatory requirements for MedTech companies?

No, GPOs themselves do not establish new regulatory requirements. However, their contracting processes and specifications can influence how MedTech companies demonstrate compliance with existing regulations, particularly in areas like quality, safety, and supply chain management.

How do GPOs impact a MedTech company's quality system?

Are GPOs considered 'economic operators' under the EU MDR?
