# Field Clinical Engineer

Manufacturer representative providing clinical and technical support in the OR or cath lab.

Reviewed by [Christian Espinosa, Founder, Blue Goat Cyber](/authors/christian-espinosa) Last reviewed May 5, 2026

## Definition

Field Clinical Engineers (also called Clinical Specialists or Therapy Support Specialists) provide in-procedure technical and clinical support for complex devices - programming pacemakers, supporting [structural heart](/terms/structural-heart) deployments, calibrating navigation systems.

What the regulation says

The FDA does not specifically regulate Field Clinical Engineers as a distinct role, but their activities fall under the purview of regulations governing medical devices, such as 21 CFR Part 820, [Quality System Regulation](/terms/qsr), which addresses [design controls](/terms/design-controls), servicing, and installation. Their interactions with medical devices and healthcare professionals are also subject to regulations concerning proper device use and promotion, as outlined in FDA guidance documents on promotional materials and [off-label use](/terms/off-label-use). The EU [MDR](/terms/mdr-reporting) (Regulation (EU) 2017/745) similarly expects manufacturers to ensure that personnel involved in device support are adequately qualified and that their activities do not compromise device safety or performance, particularly regarding [post-market surveillance](/terms/post-market-surveillance) and vigilance requirements.

## What this means in practice

FCE coverage models drive sales force economics and influence procedure adoption. Anti-kickback boundaries on what FCEs can provide ('practice of medicine' line) are continually tested.

## Examples

- An FCE provides on-site technical guidance to a surgeon on the proper calibration and use of a robotic surgical system during a procedure, ensuring the system operates according to its validated specifications.
- An FCE assists a hospital biomedical engineer in troubleshooting a software issue on an implantable cardiac device programmer, following the manufacturer's approved diagnostic and repair protocols.
- During a new device rollout, an FCE trains clinical staff on the user interface and alarm settings of a novel continuous glucose monitoring system, demonstrating features within the device's intended use.

Common pitfalls

- • Defining the line between providing technical support and engaging in the "practice of medicine" is a critical pitfall, potentially leading to legal and regulatory issues.
- • Inadequate training or understanding of regulatory requirements, such as Good Documentation Practices (GDP) or adverse event reporting, can result in non-compliance.
- • Failure to properly document all activities, particularly service and support provided, can create gaps in the device's traced history and hinder post-market surveillance efforts.
- • Permitting uncon rolled modifications or adjustments to devices by FCEs outside of validated procedures can lead to device malfunction and patient harm.
- • Lack of clear internal policies regarding the scope of FCE responsibilities and interactions with healthcare professionals can lead to inconsistent practices and compliance risks.

## Frequently asked questions

Are FCEs required to be licensed medical professionals?

Generally, no, FCEs are not typically required to hold a medical license. However, their scope of practice is limited to technical device support and may not include making medical judgments or providing direct patient care.

What kind of training should FCEs receive from a regulatory perspective?

How do adverse events involving FCEs get reported?

What cybersecurity considerations apply to FCEs?
