# Contract Research Organization

Service organization providing clinical trial management and supporting services to sponsors.

## Definition

A Contract Research Organization (CRO) delivers protocol design, site management, monitoring, biostatistics, regulatory submissions, safety reporting, and data management on behalf of device,  [IVD](/terms/ivd), biotech, and pharma sponsors. MedTech-focused CROs differ from drug CROs in their handling of device-specific endpoints,  [IRB](/terms/irb)/EC strategy for  [IDE](/terms/ide)/ [CIP](/terms/cip) studies, and reimbursement-grade evidence design. 

What the regulation says

Regulators recognize CROs as essential partners in clinical research. For example, FDA regulations in 21 CFR Part 312 (for drugs) and 21 CFR Part 812 (for devices) outline the responsibilities of sponsors and investigators, which often extend to CROs when these organizations perform study-related duties. The EU  [MDR](/terms/mdr-reporting) (Regulation (EU) 2017/745) also implicitly covers CRO activities by imposing stringent clinical evidence requirements on device manufacturers, who may delegate tasks to CROs.

Sponsors choose CROs based on therapeutic-area expertise, geographic site footprint, and budget - and increasingly on familiarity with EU  [MDR](/terms/mdr-reporting)/ [IVDR](/terms/ivdr) clinical evidence expectations.

-   A MedTech startup developing an AI-powered diagnostic delegates its pivotal clinical trial execution, including site selection, data management, and biostatistics, to a specialized CRO to meet FDA 510(k) submission requirements.
-   A large medical device manufacturer contracts with a CRO to manage its post-market clinical follow-up (PMCF) studies across multiple European countries to satisfy EU MDR Annex XIV Part B requirements.
-   An in vitro diagnostic (IVD) company partners with a CRO to conduct a performance study for a new immunoassay, ensuring adherence to ISO 20916:2019 (In vitro diagnostic medical devices - Clinical performance studies using specimens from human subjects - Good study practice) and other relevant IVDR requirements.

Common pitfalls

-   • Failing to establish clear lines of responsibility and communication between the sponsor and the CRO can lead to regulatory non-compliance. 
-   • Assuming that a CRO is solely responsible for regulatory compliance without adequate sponsor oversight is a common pitfall. 
-   • Not performing due diligence on a CRO's quality management system or therapeutic area expertise can result in compromised study data or delayed submissions. 
-   • Lack of a comprehensive written agreement detailing all delegated tasks and responsibilities can create disputes and regulatory scrutiny.
